Pigmentation on breast skin resulted from use of BRAVA non surgical breast augmentation device. The BRAVA applies suction to surface of breast and must be used daily for extended period of time. Although she may have had improved breast shape with breast lift, this would have added scars to breasts along with other additional risks. Unless certain of the need for a formal breast lift procedure added to augmentation with implants, Dr. Laverson will often try to lift the breasts as much as possible with implants (augmentation) alone to avoid the scars, extra cost, and added risks associated with breast lift. From the side view, it can be appreciated that she achieved some moderate breast lift with the augmentation alone, and this was satisfactory to her. Patient Details: She has a nice breast shape for augmentation, round with nipples well centered. For breasts with this shape and a well defined lower breast crease, Dr. Laverson most often hides the scar in this crease, and selects round implants with a mid-range or high profile. Women requesting only moderate augmentation may be best served by low or moderate profile implants. Most currently available implants have only two dimensions, diameter and projection, or profile. Low profile implants are more pancake shaped, and high or full profile implants are more spherical or globular. Mid-range profile implants were selected because of her pre-existing full A cup and desired C cup final result. If she wanted D-cup or larger, high or full profile implants would be chosen. Patient Details: One definition of dignity is “The presence of poise and self-respect in one’s deportment to a degree that inspires respect.” The goal of most cosmetic surgery, in Dr. Laverson’s opinion, is to preserve or improve personal dignity. For many women, the motivation for breast augmentation is to improve their own self-respect, or phrased differently, confidence. Implant size, position, and quality of result is critical to achieving this larger goal. Disproportionately large implants, insufficient augmentation, or a poor overall result can detract from personal dignity. In this case, a moderate augmentation improved femininity of her shape. Although the procedure costs several thousand dollars, over the long term, the value of the effect to her, and so many women, is priceless. (more images)

Patient Details: See Case 52 for comparison. Although she has elliptical shaped breasts also, they are positioned lower on the chest. Using round implants centered behind her nipples would move the bottom portion of her breasts below her ribs onto the abdominal wall. She would feel heavy, maternal, and clothing would not fit well. She would lose her waist (hourglass figure) if breast volume were too low. Besides looking bad, carrying larger breasts lower would be burdensome and uncomfortable. Therefore Biodimensional® (elliptical shaped) implants were chosen, Allergan Style 363, 510 ml. These keep the breast volume high on the chest, and are high profile implants with good projection. Aesthetic judgment, experience, and open communication are important in implant selection. She had abdominoplasty (tummy tuck) at the same time, the “Mommy Makeover” procedure. He also chose the submuscular position to cover the prostheses with an additional layer, thus rendering them less visible as implants, and again, more natural in consistency. Implants selected and positioned slightly medially and superiorly to meet her needs, through inframammary crease. The inframammary crease approach means the implants were placed through an opening created at the lower border of the breast, within the crease at the bottom of the breasts where they meet the chest wall.
